Health Dialog enters multi-year agreement with New York-based health plan CDPHP

BOSTON — Rite Aid subsidiary Health Dialog announced Tuesday a multi-year agreement with CDPHP, a New York-based health plan that recently received the National Committee for Quality Assurance’s highest ratings for quality care. Under the terms of the agreement, Health Dialog will provide its solutions for disease management, health risk assessment, and HEDIS support to CDPHP’s more than 450,000 members. The relationship extends an existing collaboration of nearly a decade, which was designed to help CDPHP members make good healthcare decisions based on the best medical evidence available.


 


In additon to working with health plans like CDPHP, Health Dialog provides the Care Coaches that participate in the Rite Aid Health Alliance offering. 


 


Under the agreement, Health Dialog will continue to work with CDPHP to provide patients with one of the most complete approaches to disease management. CDPHP members will have access to Health Dialog’s complete library of shared decision making aids, one-on-one health coaching and personalized wellness initiatives. Combined, these programs give CDPHP members comprehensive support along the path to better health. Data from a Health Dialog member satisfaction study show that improved learning combined with interactive and collaborative discussion with both a health coach and the treating physician can help patients make better healthcare decisions suited to personal values and lifestyle.

Health Dialog’s disease management offerings focus on providing patients with the resources and support needed to change behaviors and manage chronic conditions. Through the continued relationship, Health Dialog will offer CDPHP members:


 



  • Access to Registered Nurse Health Coaches specializing in behavior change. These nurse coaches provide the decision support and knowledge that allow health plans to address member barriers and improve care while lowering costs;


  • Access to award-winning decision aids to help educate patients and their families about managing their conditions and symptoms; and


  • HEDIS support and sophisticated analytics to build and implement a strategy for improving HEDIS scores on a range of measures. The HEDIS program also supports the health plan efforts to meet CAHPS Health Plan Survey scores. Both programs consist of three key program components — identification, outreach and direct health coach intervention support.
